Waiver

Effective as of April 01, 2025

Student/Staff/Guardian Requirements:

The Safety of our students, camp staff, and families remains our top priority. We will only allow staff and students within the designated facility at the scheduled time of the Innovate Learning Hub Programs. No other individuals will be allowed in the instructional area unless pre-approved by director or designate.
Furthermore, all participants in activities at Innovate Learning Hub designated facilities must agree to and abide by the following:

    •  If the camper or parent is sick, please do not come to the facility.
    • Campers may arrive no earlier than 15 minutes before their camp starts or before their early drop-off time if service was paid for and must be picked up/leave no more than 15 minutes after their program concludes or after their late drop-off.
    • For summer camps, send your campers with lunch. Light snacks lunch will be provided.

Peanut & other food allergies:

At Innovate Learning Hub, we want all our camp to be a safe environment for our students. We have many students that have severe food allergies, so we have three simple rules:

1. No peanut-based food items may be brought to camp.
2. No sharing of food brought to camp except by siblings, &
3. No outside food may be brought in to celebrate a birthday.

Great alternatives to celebrate a student’s birthday are pencils, erasers, rubber balls, stickers, etc.

Thank you for following these simple rules and making all our camps safe and inclusive for all our students!

Photo Waiver:

Registrants and participants of camps, programs and special events permit Innovate Learning Hub to take photos and videos of themselves and their children for publication in the program brochure, web site and additional uses as Innovate Learning Hub deems necessary unless the registrant or participant expressively files with Innovate Learning Hub a written objection as to photos or videos of themselves and/or their children. Written objection must be sent to: info@innovatelearninghub.com with the subject “Photo Waiver – your last name”

WAIVER, AGREEMENT NOT TO SUE AND INDEMNIFY:

Please read this form carefully and be aware that in registering yourself or your minor child/ward for participation in Innovate Learning Hub programs, you will be waiving and releasing all claims for injuries you or your child/ward might sustain arising out of said programs.

Innovate Learning Hub strives to conduct its camp, programs, and other activities in a safe manner, and holds the safety of participants in the highest regard.

I recognize and acknowledge that there are certain risks of physical injury to participants in the above program(s) and I agree to assume the full risk of any such injuries, damages, or loss regardless of severity which I or my child/ward may sustain because of participating in any activities connected or associated with any such program(s). Innovate Learning Hube continually strives to reduce such risks and insists that all participants follow safety rules and instructions which have been designed to protect the participant’s safety. I waive and relinquish all claims I or my child/ward may have against Innovate Learning Hube and its officers, agents, servants, and employees because of participant in any of the above program(s). I hereby fully release and discharge Innovate Learning Hube and its officers, agents, servants, and employees from all claims from injuries, damage, or loss which I or my child/ward may have, or which may accrue to me or my child/ward on account of my participation or the participation of my child/ward in any of the above program(s). I further agree to indemnify and hold harmless and defend Innovate Learning Hub and its officers, agents, servants, and employees from all claims resulting from injuries, damages and losses sustained by me or by my child/ward, and arising out, connected with, or in any way associated with the activities of any of the program(s).

I have read and fully understand the program details and waiver and release of all claims.

I understand that by signing below, I am acknowledging my AGREEMENT TO THE ABOVE.

Signing below confirms that you have read the above camp information, acknowledge, and accept the entire content of the preceding information, and that you agree to be bound by it.

By agreeing and signing here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address to contact you regarding any changes, if necessary.
